About our programs

Diakon Community Services for Seniors provides community-based services to seniors. We are a program of Diakon Lutheran Social Ministries

Meals on Wheels, an outreach ministry here in Schuylkill County that services homebound seniors helping them to remain independent and in their own homes. We work in cooperation with The Office of Senior Services to deliver 5 meals per week to over 500 clients. These meals are delivered Monday, Wednesday and Friday.

Senior Community Resource Centers in Schuylkill County promote the social, emotional, physical and nutritional well being of older adults through a variety of community-based services and activities. Senior Centers currently serve approximately 270 congregate meals in addition to providing County seniors with help completing PACE applications, obtaining STS bus passes etc. The program is funded in part by the Office of Senior Services.

PrimeTime Health provides health information to older adults in a way that is upbeat, encouraging and fun. The program, which works in conjunction with many existing programs and services, reaches hundreds of seniors in Schuylkill County through health screenings, exercise programs and nutrition updates. The program is funded in part by the Office of Senior Services.

Apprise offers counseling on Medigap, Medicare, Managed Care policies, and prescription drug plans to seniors in Schuylkill County. Trained volunteers provide counseling on PACE applications, change of requests on Medigap policies, answer questions on managed care and help to file appeals on health insurance denials. The program is funded in part by the Office of Senior Services in Schuylkill County.

Apprise Telecenter is a state-wide hotline manned by volunteers that is operational Monday through Friday 9am to 4pm.. These volunteers answer basic medicare, medigap etc. questions. Schuylkill County volunteers man the Telecenter on Wednesdays from 9am to 4pm.
