The Program of All Inclusive Care for the Elderly (PACE) is an initiative supported by Medicare and the North Carolina Medicaid program. It's designed to deliver innovative care and social services to qualified participants aged 55 and older.
PACE@Home offers comprehensive and holistic care to the seniors we serve. Our goal? To enhance life by nurturing the daily capabilities that allow seniors to remain active in their homes.
PACE@Home helps people with long-term care needs enjoy their independence and works with them to obtain all necessary health car services. We provide ourselves on promoting the social, physical, and mental well-being of each individual.
All PACE@Home participants benefit from ongoing care, access to an adult day center, and regular monitoring by a team of doctors, nurses, social workers, therapists, personal care workers, and other dedicated staff.
PACE@Home delivers all necessary medical and supportive services that enable seniors with chronic care needs to maintain their independence in their own homes. These services icluded: adult day health services, medical care and medical specialist, dentat, vision, hearing, and foot care, meal planning and nutrition services, skilled nursing, including home helath care and personal care (e.g., assistance with bathing and dressing), prescribied medicationsm, social services, respist care, physical therapy, occupational therapy, and speech therapy, recreational and social activiities, transportation to helath-related appointments and the PACE@Home center, hospital care, emergancy services and nursing facility care.
