Mission Statement

It is the mission of Brookline Health Care Center to be a community leader providing quality driven Post Acute/Long Term Care Services.

We are committed to providing a continuum of care which includes a warm and homelike enviroment that promotes optimal functional ability, interdisciplinarycare and the integration andm provisions of community resources and education. It is our intent to provide these services without regard to sex,race,creed, national origin,age and disability or payer class.

We promote a challenging and dynamic proffessional work enviroment through ongoing staff education and Continous Quality Improvement programs, which meets and or exceeds the needs of an ever changing health care population.

Description

Providing in-patient medical,nursing rehabilitative care for those individuals who are unable to live in an alternative community setting, but who do not need the diagnostic or surgical services normally provided by acute care hospitals.

Improving the quality of life for our residents, by assisting them in acheiving their maximum level of function and independenc. This is accomplished thruogh supporting each residents psychological and social needs, maintaining their self dignity, providing a home-like enviroment, and respecting residents need, Bill of rights and privacy.

Establishing a comprehensive pain management approach to care and recognizing that residents have a right to expect a quick response to reports of pain.
