Our community is committed to improving lives everyday by igniting hope, instilling pride, offering compassionate support for adults with disabilities and their caregivers. And in so doing, we will promote a more civil and humane society.

We aim to broaden awareness of the needs of adults with disabilities through expanding partnership, public advocacy, and membership.

A Grace Place is dedicated to providing individuals with person care-centered care that promotes choice, purpose and meaning in everyday life.

Description

A Grace Place Adult Care Center provides services to adults, 18 years and older, with disabilities and age-related conditions and their caregivers. The agency serves a more severe population than most, in terms of the mix of age and diagnoses and the percentage needing assistance with the activities of daily living. More than 90% of the individuals we serve are living at or below the federal poverty level.

A Grace Place Adult Care Center offers day programs that provide opportunities for socialization, cognitive and intellectual stimulation, basic health maintenance and assistance with daily living activities. We currently serve 230 participants each day.
