Providing quality end of life care involves a number of professionals working together, from doctors and nurses to social workers, dietitians, and chaplains. Sanctuary Hospice brings together an experienced team to celebrate the lives of those we serve and to deeply care for the particular medical, emotional, spiritual, and practical needs facing our patients.
The Sanctuary Hospice Team:
Medical Director--makes home visits, provides support, participates in PTS plan of care, consults with clinical team, and oversees patient care.
Physicians--available for consultation and home visits.
Hospice Nurses--educate patients and families about care; monitor medications, vitals, and provide all nursing care.
Social Workers--provide resources and support.
Certified Nursing Assistants (CNAs)--provide personal care, assist with bathing, grooming, light meal preparation, and light housekeeping.
Chaplains--available for spiritual support, prayer and guidance, or act as mediator between patients and as a spiritual counselor.
Dieticians--complete nutritional assessments and offer recommendations.
Trained Volunteers--offer personal companionship and allow for caregivers to have some respite during volunteers' visit.
