Mission Statement

REACH, Reaching Elders with Additional Community Help, bridges the gap between already existing community services to better assist elders and their families to access available help.

Description

REACH brings diverse service providers together to address a specific need through a formal communication process.

REACH uses a simple 1-2-3 approach. Community services such, as a local Rescue EMT, will:

  1. Identify an elder in need of community help;
  2. Refer that elder to REACH partner Council on Aging by completing a REACH Referral indicating possible unmet needs an elder may have.

  3. Fax the Referral to REACH and the COA to trigger a timely response. Once the REACH Referral is received , REACH and the COA may, if appropriate, mobilize additional community resources and dispatch quick response trained volunteers.
