REACH (Reaching Elders with Additional Community Help)

Description

REACH brings diverse service providers together to address a specific need through a formal communication process.

REACH uses a simple 1-2-3 approach. Community services such, as a local Rescue EMT, will:

1. Identify an elder in need of community help;

2. Refer that elder to REACH partner Council on Aging by completing a REACH Referral indicating possible unmet needs an elder may have.

3. Fax the Referral to REACH and the COA to trigger a timely response. Once the REACH Referral is received , REACH and the COA may, if appropriate, mobilize additional community resources and dispatch quick response trained volunteers.
