Description

We are a "one stop shopping" for elders and disabled individuals over 22 years old, providing case management services designed to help them live independently in their homes for as long as possible. We provide non-medical services that are not covered by Medicare or Medicaid that allow elders to remain at home. A case manager will set up an appointment to visit the person in their home to assess for eligibility and develop a service plan. If not eligible for home care services other recommendations will be made, including private pay services. Services are authorized from contracted vendors, as we do not provide direct services. An example of services are home delivered meals, shopping service, personal care, laundry, etc.
