Horizons Hospice, LLC is guided by a tradition of personal, clinical and technological excellence. We believe that serving patients in the community setting is the most important activity of the organization.

We are dedicated to providing the highest quality of patient care with compassion and respect for each person.

The organization’s programs and services will reflect its written mission as stated in this policy. Published statements will be available to personnel, patients, and the general public. These statements will be reviewed and revised as needed and approved by the governing body and least every 36 months. Horizons Hospice, LLC senior management personnel will continuously monitor employee performance to ensure it supports the organization’s mission.

Horizons Hospice, LLC is driven by the philosophy of commitment to our patients, leadership and excellence as defined below. We recognize the unique physical, emotional, and spiritual needs or each person. We strive to extend the highest level of courtesy and service to patients, families/caregivers, visitors, and each other.

We deliver the state-of-the-art hospice services with identified centers of excellence. We engage in a wide range of continuing education, clinical education, and other programs for professionals and the public.
