Mission Statement
Home Avenue Adult Day Health Care Center seeks to expand health care and human services to provide for the growing needs of the frail and elderly, Alzheimer's, mentally ill and chronically ill, and developmentally disabled population of the San Antonio Service Area of Los Angeles County.
This population is classifed by traditional, fragmented health care systems as high risk and high cost. Consequently, this target population is a priority for public, private and public/private health care providers to develop truly cost efficient and cost effective care.
The vision is: to grow a health care enterprise that is flexible and alert to the constant changes and trends in the health care industry. To resopnd to the growing needs of a growing segment of the human community.
Home Avenue Adult Day Health Care Center objectives are:
To restore or maintain optimal capacity for self care by frail and elderly persons (age 55 or older) in its Los Angeles County Service Area communities.
To delay or prevent inappropriate institutionalization of these persons.

Who Are We?---Home Avenue Adult Day Health Care is a day program providing a variety of health, therapeutic, and social services designed to serve the special needs of frail and elderly persons as well as adults who are disabled.
What do we do?---Home Avenue Adult Day Health Care has a multidisciplinary team of health care professionals who conduct a comprehensive assessment of each potential participant to determine which ADHC services meet each individual's specific health and social needs. Our therapeutic environment is designed to promote socialization and to maximize each individual's potential for independence and self-esteem.
Basic Services: Individual Needs Assessment, Nursing Care, Rehabilitative Therapies, Social Services, Recreational Activities, Transportation, Meals and Snacks, Family Consultation.
Additional Services: Podiatry, etc.
Hours: Monday through Friday 9:00am-2:30pm
