Mission Statement

The mission of SouthernCare is to assure that residents of its service locales have access to a program of care that enhances the quality of life for the terminally ill and their families. Care of the whole person is emphasized by providing physical, spiritual, and emotional support to all medically appropriate persons, without discrimination. Efforts are centered on meeting needs by offering highly individualized compassionate care in the person's home or home like setting. SouthernCare recognizes the value of dedicated staff and volunteers and affirms each as individuals, as well as members of the care team. SouthernCare is dedicated to enhancing the care of all in the community by education, and by example, serve as a symbolic reminder that life is to be lived until you die.

Description

At SouthernCare, our goal is to ensure quality of life to patients who are dealing with limited life expectancy. However, we do not have a pre-conceived, standard program that applies to everyone. After all, people are different. So we listen, we learn, and that's what makes us different.


SouthernCare Hospice serves clients and families in 16 counties in south central Wisconsin including Adams, Columbia, Dane, Dodge, Green, Green lake, Juneau, Jefferson, Iowa, Lafayette, Marquette, Richland, Rock, Sauk, Waukesha, and Walworth.

Each patient that comes to SouthernCare receives specialized, unique care. That includes much more than just exeptional medical attention. At the heart of SouthernCare is our staff. From this diverse group of healthcare professionals, we create what we call the Interdisciplinary Group for each patient.
