Mission Statement

Our Mission is to offer best practices in hospice and comfort care to our patients and their loved ones as they face the physically, emotionally, spiritually and financially challenging times that can accompany end-of-life, so patients can truly live and celebrate life on their own terms.

Description

Our Vision is to be recognized as the hospice of choice for families and physicians when individuals are faced with a terminal illness and elect to live the remainder of their life in the comfort of their own home. Through our compassionate team of licensed and certified professionals, we provide the highest standard of care to our patients, and ongoing support to their loved ones, so the patient is able to truly live the rest of their life to the fullest. Our comprehensive care extends beyond the life of our patients, offering bereavement support to anyone who has experienced the loss of a loved one.

Our Values are rooted in providing the best care possible to our patients and their families. In every way we provide service, we embrace compassion, communication, professionalism, and customer service:

First Choice Home Health & Hospice is mission oriented and cares very much about the people it serves. The volunteer opportunities are varied and flexible. It provides a welcoming environment and the Volunteer Coordinator is: engaging, friendly, and stays in regular contact with the volunteers. There are also opportunities for the volunteers and employed staff to meet one another and that helps to build a feeling of community.
