Embrace Hospice uses a team approach to care for the physical, spiritual, emotional, and social needs of the patient and their family. This interdisciplinary method means all staff members work together to meet goals and care for the patient. The patient and family always remain in control of the plan of care, with their nurse serving as their team leader.

It is important to realize that Hospice is a special kind of care designed to provide sensitivity and support for people in the final phase of a terminal illness. Hospice care seeks to enable patients to carry on an alert, pain-free life and to manage other symptoms so that their last days may be spent with dignity and quality at home or in a home setting
