Mission Statement

To improve care for those facing life-limiting illness through a commitment to provide comprehensive, competent, quality care which optimizes comfort and dignity and is consistent with the patient and family needs and goals, with the patient's needs and goals as priorities while maiantain ethical standards of business and critical practice.

To deliver timely, well-coordinated, family centered end-of-life care including bereavement and counseling services.

To provide and case manage services that are consumer oriented.

To ensure continuity of care, consistent with the needs of the patient/family/caregiver as they relate to the terminal diagnosis, culture, environment and appropriate level of care.

To raise awareness relating to palliative care through public education and advocacy.
