Diabetes Partnership of Cleveland
Diabetes Partnership of Cleveland's mission is to "ensure that Greater Clevelander affected by diabetes have the support they need to live well with the disease."
Description
Diabetes Partnership of Cleveland works to educate people with diabetes about self-management to give them the skills to prevent the overwhelming complications of the disease so that they are able to live healthy, productive lives. Diabetes Self-Management Education does not replace medical care, but complements care in assisting clients with their responsibilities in changing or sustaining health behaviors. Diabetes Partnership of Cleveland believes that every person deserves access to high quality health care, particularly fundamental diabetes education and support services and Camp Ho Mita Koda for children with diabetes.
The administrative office is located in Beachwood, but services are provided throughout the Greater Cleveland region. Camp Ho Mita Koda for children with diabetes, is located in Newbury (Geauga County).
