Description
Horizon Hospice is the third oldest hospice in San Diego County and the first in the community to offer care in a nursing facility setting as well as in private homes.
Horizon's founders include four generations of a family born and raised in San Diego whose interest in hospice philosophy was sparked after personally caring for several family members through the terminal stages of illness. This personal experience as a family in crisis, as well as a desire to give back to their comunity, led to the creation of North County Hospice in 1984. In 1986 North County Hospice reincorporated as Horizon Hospice. Today, Horizon remains the only Medicare certified hospice covering EVERY COMMUNITY in San Diego County.
All of the policies and practices of Horizon Hospice are influenced by our history of being family owned and operated. We believe it is an honor to provide compassionate care to people in the last stages of their lives and a privilege to help the patient's family take care of their loved ones.
