Mission Statement

Tri-County Health Network, incorporated in 2010, as the healthcare arm of the Telluride Foundation, is a nonprofit entity committed to improving the quality and coordination of health and healthcare services in the our rural three county region of southwest Colorado by increasing access to healthcare and integrative health services at lower costs through collaboration and innovation.

Description

The Tri-County Health Network is a collaboration of the six leading medical and mental health providers in our tri-county region created to increase quality, accessibility and coordination of programs and services for improved healthcare in our rural, underserved region.

The Network’s strategic plan is two-fold: one to continue to identify, develop, and operate Community Outreach Programs that will improve access to healthcare in our region; and secondly to work with the provider community in developing initiatives focused on quality that will have a positive impact in improving the overall health of our community memebers while decreasing cost.
