Our goal is to facilitate continuity of care between the patient's physician, the Hospice physician, the patient and their family and caregiver. We believe maintaining quality of life by supporting personal decisions, cherishing meaningful experiences and to preserve the dignity of the patient wherever they are in life's journey.

Description

Hospice is not a place, it is a concept of care for those who have a life limiting illness. Even when medicine cannot provide a cure, it can offer comfort, care and assistance that help maintain a better quality of life for the patient. This type of attention is called palliative care and involves the aggressive treatment of physical and emotional pain and symptoms. If focuses on enhancing a patient's comfort and improving that person's quality of life.

Hospice affirms life and neither hastens nor postpones the end of life. It is predicted on the belief that through sensitivity, appropriate care and the support of a caring community, terminally ill patients and their families can begin to prepare, both mentally and spiritually to face the challenges of life's journey.

Home Health Care Professionals & Hospice recognizes that psychological, spiritual and social services, in addition to medical and clinical care, can contribute to an improved quality of life for patient's and their families.

Every patient is served by an entire team of dedicated professionals who are guided by the choices and decisions made by the patient, the family and the physician.

Hospice services are available 24 hours a day / 7 days a week on an intermittent basis. Hospice care can be provided to you at your home, skilled nursing facility, assisted living facility, personal care home or hospital.
