Mission Statement

CReATE, established in 2007, refurbishes mobility assistive technology (AT) and makes it available to people who need assistive technology at a low cost.
CReATE is a place where Utahns can GET or DONATE used mobility assistive technology.

Description

Getting a Device:

  1. Call CReATE at 801-887-9398.
  2. Make an appointment for an assessment to determine the type of device needed.
  3. Fill out the proper paper work.
  4. Pay the service fee, which is based on the time spent to repair and clean it.

  • Power wheelchairs
  • Manual wheelchairs
  • Power scooters
The availability of specific devices is dependent on the current inventory. To find out what is available and make arrangements to acquire AT or to donate, call 801-887-9398.

Donating a Device:
  1. Contact us to determine if the device can be accepted by CReATE.
  2. Deliver the device to CReATE.
  3. Device donations are tax deductable.
Who CReATE Serves: Services are provided to people with disabilities throughout the state of Utah. CReATE will accept consumers regardless of race, national origin, religion, geographic location within the state of Utah, age, sex, disability, political affiliation, or belief. No person shall be excluded from participation in, be denied the benefits of, or be subjected to discrimination by CReATE. The CReATE staff reserves the right to deny services to any consumer when the severity of the disability or complexity of the technology needs of the individual is beyond the scope and training of the CReATE staff. CReATE device transfer services are limited to refurbished devices in the current inventory.
Transferring Device Ownership: CReATE will not loan equipment. The ownership of the device will be transferred from the CReATE Program to the individual acquiring the AT device. CReATE’s intent is to transfer AT devices to individuals and therefore the devices are not for resale.
Devices are not sold but a service fee is charged based on the work performed on the chair so that it will meet manufacturer specifications.
