Riverside PACE's mission is to enhance life for frail older adults by providing compassionate quality care and services.

Riverside PACE strives to achieve this mission by empowering participants to live in their homes and community for as long as it is medically and socially safe.

Riverside PACE is designed to preserve and support the participant's family unit while maximizing the dignity of, and respect for, the participant. Riverside PACE remains committed to comprehensive, coordinated, personalized quality of care to all who receive our services.

Description

Riverside PACE is an innovative program of comprehensive care that is based on the OnLok model of long-term care introduced in San Francisco more than 30 years ago.

Here at Riverside PACE, a team of doctors, nurses, social workers, health aides, home care coordinators, transportation providers, rehabilitation therapists, dietitians, recreation therapists and others work together to deliver comprehensive services, activities and programs.

Working with PACE participants and their families, this team develops a personalized plan that is tailored to that person's medical and social needs, which aims to avoid health problems before they happen.
