Guardian Adult Day Health is a nonprofit Center that represents an alternative to premature institutionalization for the disabled adult or frail elderly in need of daily medical attention. The mission of the Center is to promote the well-being of impaired individuals living in the community, in order to maintain them at or restore them to their optimal functional potential and to help them remain in or return to their homes. Our goals are to foster independence of participants and to enhance the quality of their lives.
Additionally, our goal is to work toward fulfilling the following needs of our participants:
- The need to be considered a real part of the community.
- The need to occupy much expanded free time in more satisfactory ways.
- The need to render some socially useful service.
- The need to enjoy normal companionships.
- The need for recognition as an individual.
- The need to have opportunity for self-expression.
- The need to have health protection and care.
The Center serves frail adults who experience physical, mental or social problems associated with stroke, isolation, confusion or other conditions. It provides them with the opportunity to meet new people, participate in challenging activities, and enjoy daily life in comforting surroundings. We are committed to treating our participants in a caring and thoughtful manner and providing our staff with a positive and challenging work environment. As the only adult day health care center that serves this economically depressed area of Western Contra Costa County, we are looking forward to the possibility of partnering with you in our shared mission to maintain and improve the health of our community. Our staff includes 2 RNs, 2 social workers, activity coordinator, OT,PT, and LCSW. Our services include transportation to and from the center , breakfast lunch and a snack.
