We are a licensed Home Health Agency, fully accredited by The Joint Commission We specialize in Care Coordination, Skilled Nursing & Non-medical assistance at home such as: -Concierge/Care Management -Homemaker/Companion: - Fall risk assessment and prevention, meal preparation, light housekeeping, laundry, medication reminders and assistance, companionship, and transport services to doctors appointments and other routine errands -Home Health Aid / Personal Care / Respite: - Bath and shower assistance, ambulation assistance, dressing and grooming services, and incontinence care -Skilled Nursing and Therapy: - Post op care, Physical, Occupational and Speech Therapy
