Gardendale Hospice

Description

Gardendale is one of only a few hospice agencies CHAP accredited in several counties within our service territory.

A person is eligible for hospice when their primary physician and our medical director certify a terminal diagnosis with a prognosis of approximately six months to live. It is important that a patient/client and their support systems embrace palliative care rather than curative care. A successful hospice program will always support all aspects of patient care to include the emotional, the spiritual, and the physiological. The Medicare Hospice benefit covers all services specific to hospice care. Our administrative staff can pre-authorize services with your individual health care plan prior to the initiation of services. Medical professionals, friends, and family members or patients/clients can make a referral to our hospice. We are committed to provide a free evaluation within 24 hours of a referral, once eligibility criteria has been determined.

Gardendale Hospice is locally owned and operated and is licensed and certified by the State and Federal Medicare/Medicaid program. Gardendale is accredited by Community Health Accreditation Program (CHAP).
