Care Coordination/Home Telehealth is a model of care that employs licensed care coordinators to facilitate enhanced healthcare access and implement processes to ensure healthcare continuity for veterans. Disease management strategies combined with informatics and telehealth technologies are used to support this process for many of our veterans in their homes .
