Our mission is to improve the health of children and adults, particularly vulnerable populations, through education and connections to health care and support services.
Description
History
Health Net of West Michigan is a non-profit, 501(c)(3) organization formed in April 2014 from Kent Health Plan and incorporated programs from First Steps Kent including: Kent County Oral Health Coalition (KCOHC), FitKids360, and Community Healthcare Access Program (CHAP). The organization was formed to shift focus after the implementation of the Affordable Care Act and initiation of the Healthy Michigan Plan.
How we helpWe work to bridge gaps in care by connecting community members to health systems, providers, and social services. We assist individuals one-on-one with acquiring services that directly and indirectly affect health, along with health education services on-site and at partnering organizations.
MissionOur mission is to improve the health of children and adults, particularly vulnerable populations, through education and connections to health care and support services.
VisionOur vision is for a community in which all people have the knowledge, resources, and support to maintain optimal health for themselves and their families.
