We provide high quality health care programs and related support services in a community setting throughout Franklin County, Vermont. We are committed to excellence in service. We are committed to providing medically necessary care to all area residents who need it regardless of their ability to pay. Description

We provide home health and hospice services to people of all ages in Franklin County, VT. Our programs include rehab therapies for children and adults as well as a full range of skilled nursing and long term care services to meet your needs.

Our specialized care for people with chronic conditions includes telemonitoring and our Better Breathing program to help people cope with chronic lung conditions.

Our Nurse-Family Partnership program pairs a nurse home visitor with a first-time mom during her pregnancy and follows the mom and her baby through the child's second birthday.

Our hospice services include pediatric palliative care and a children's bereavement program to help children of our hospice patients deal with the loss of a loved one. We provide palliative care services to adults who are living with life-limiting illnesses to provide the comfort and care they need at home.

Our Hospice Volunteers provide respite to families and companionship to patients in our hospice program. These volunteers also provide invaluable support of our fundraising and other efforts. For instance, a group recently created a new set of memory boxes and other resources for our children's bereavement program.

Our clinicians and staff include social workers, nurses, rehab therapists, nurses, licensed nursing assistants and care attendants.
