Mission Statement

Tri-City Health Center (TCHC) is committed to delivering excellent health services in a caring, nurturing, and respectful atmostphere; we promote optimal health, eliminate preventable disease, and improve the quality of life for every individual and family in our community. We believe great health care means attending to the patient's unique medical and cultural needs. We are the medical home to more than 22,000 area residents and make quality, affordable health care available to everyone who needs it, regardless of their ability to pay.

Description

Tri-City Health Center (TCHC) was originally founded in 1970 as a family clinic for low-income minority women living in southern Alameda County, including the communities of Fremont, Union City and Hayward. TCHC became a Federally Qualified Health Center (FQHC) in 2003 and now offers primary medical, dental and behavioral health services for the entire family through nine sites, including 4 Fremont-based primary care sites, a dental site and a mobile clinic.

TCHC is the only FQHC in the City of Fremont offering a health care home which emphasizes disease prevention and chronic disease management. TCHC’s comprehensive menu of services include: primary care, pediatrics, prenatal, women’s health, HIV/Hep C, chronic disease management, nutrition counseling, family planning, behavioral health, dental, vision, care coordination, transgender health, and health insurance counseling and enrollment. Recognizing the rich diversity in our community, Tri-City proudly offers comprehensive services and specialty care programs delivered by a capable and caring staff that speaks 20 different languages.

Over the years, we have stayed faithful to our mission of providing quality healthcare services to the underserved and uninsured families in our community, without regards to financial position, ethnicity, language, culture, sexual orientation, or the ability to pay.

Would you recommend Tri-City Health Center?

First of all,I'm homeless and facing difficulties in life.no insurance I quit my job.and I'm having a health issue.I don't know where to go to seek for help coz me myself I can't believe that this is happening to me.I walk in to tri city health center scared nervous and confused.I ask one of the ladies in the front desk and I told them that I'm homeless and I don't feel good because.I'm having a high hypertension.they are ALL nice to me.and they treated me like a good friend.they put me in a homeless program.I have my meds.my check ups and I hope and pray,with the help of all the people in tri city health center.I SALUTE YOU ALL.THANK YOU FOR ALL OF YOUR KINDNESS AND COMPASSION.I also want to thank you,Dr.Ramshandani(i call her Dr.R):-)Dr.blau ms.cynthia,the beautiful ladies of the front desk.ms.tisa hall,and Dr.love (not her real lastname)you know who you are.the first-time doctor who treated me that first day.I owe you a lot ma'am.from the button of my heart thank you
