Hidalgo medical services

Description

HMS is a commuity driven medical/social model providing comprehensive primary medical and dental services with clinical sites in Animas, Lordsburg, Silver City, Bayard and the Mimbres Valley. Clinical services are supported by a broad range of social services. These are primarily staffed by community health workers or promotores and lincensed social workers. Client and outreach srevices include WIC, Families First, medicaition assistance, facilitated program eligibility assistance, community-based and remote location screening and education, home visits, care and case management, and more recently, mental health srevices.
