Mission Statement

Calvary has been committed to the non-abandonment of its patients and has focused its mission on meeting the emerging physical, psychosocial and spiritual needs of both patients and families. Our Calvary Hospice @ Home services are part of the continuum of care for the terminally ill. We provide the same standard of excellence to those patients whose illness can be managed at home regardless of the diagnosis.

Description

Patient care for Hospice takes place in the patient's home, the nursing home. assisted living facilities. If a patient is appropriate for inpatient care, we can bring them into Calvary Hospital or one of our contracted inpatient facilities. Hospice @Home provides care to patients in the Bronx, Manhattan, Queens , Brooklyn, Westchester, Nassau and Rockland counties. We provide the patient and family with visits from the nurse, social worker, spiritual counselor, and volunteer. The Hospice Volunteer Program at Calvary Hospice @ Home is dedicated to providing our patients and their families with the emotional support needed to cope with the difficult issues surrounding death and dying. Volunteers provide companionship and respite care to the patient and family in their homes or in the other hospice settings.

Dedicated Team! The staff is so greatful for our contributions, everyone is so friendly and our volunteer coordinator is so attentive, caring and sweet! The patient and families are so appreciative for any time you can give. It makes being out there with the patients in the community worth it. Aaaand the patient and families need you so join me; because it only take a little to give a whole lot!
