ESTABLISING RELATIONSHIPS with patients for ongoing management of chronic diseases, such as diabetes, cardiac disease, pulmonary disease, etc. EDUCATING THE UNISURED to enable them to play an active role in their ongoing wellness. ASSISTING THE UNINSURED in breaking the cycle of costly and effective emergency room care. NURTURING RELATIONSHIPS with patients that lead to wholeness, wellness and healing. BUILDING RELATIONSHIPS with the faith community to assist in caring for those who are sick and in need.

Description

Grace Medical Clinic is here for: • Those who are working but have no insurance. • Full-time students with no insurance. • Individuals who are between jobs. Patients will be seen on an affordable sliding scale based on household income and the number of dependants in the family. In addition to a full-time, on site, board certified physician, Grace Medical Clinic has partnered with others to provide patients with access to medical specialists and diagnostic services such as imaging, laboratory testing, and inpatient services. The Clinic will provide guidance to patients in obtaining assistance to pay for medications needed to treat the medical condition. Pastoral care will be available upon request.
