Opportunity Description ABOUT US: Established in 2011, our mission is to improve health outcomes and quality of life for individuals by addressing the social determinants that influence health and wellness through education, training, advocacy, and programs. POSITION: Certified Community Health Worker Trainee (CHW) HOURS: Must work at least 20 hours per week AND attend training for 20 hours/week for six weeks (online options offered). After training the schedule will be flexible/TBD. IMPORTANT: This position is available via a training program that will prepare the chosen candidate to sit for the state certification exam. A monthly stipend of up to $1,000 will be provided. Upon completing the program requirements, the candidate will be provided with support services and financial assistance to sit for the certification test. ABOUT THE POSTION: The CHW will conduct targeted outreach (traditional and face-to-face interaction in the community and internet-based communication) throughout Central Florida to marginalized, under-resourced, and under-exposed communities and populations. The CHW will refer and link individuals to services, conduct CLIA-waived clinical tests, basic medical screenings, and will provide high-quality health education. Training will be provided. This position requires routine and ongoing community engagement. RESPONSIBILITIES AND PRIORITIES MUSTS (on the job training in these areas is required prior to hire, is not provided by us, or is minimally provided by us): MUST have a high degree of familiarity and/or knowledge of the Seminole County, FL and Sanford, FL area. MUST have experience working with or be a member of a BIPOC (Black, Indigenous, or People of Color) community/population to best ensure representation. MUST have high-level proficiency in major computer skills, internet usage, and applications such as Word, Excel, PowerPoint, social media, etc. (uses a computer and the internet to carry out personal and business-related tasks on a daily and routine basis). MUST be well-organized and able to stick to plans and commitments and complete follow-up/follow-through tasks as planned. MUST be comfortable initiating and maintaining contact with new encounters (clients, partners, team members, etc.). Conduct CLIA-waived clinical tests in compliance with strict standards of the Department of Health and other regulatory agencies. Conduct targeted outreach - both traditional street outreach and internet-based outreach. Engage individuals in their own neighborhoods or venues where they typically congregate to provide related health information, education, referrals, and linkage to services. Assist individuals in receiving social services and medical care services. Verify that services were accessed post-referral. Distribute educational materials, condoms, and other medical tests and supplies as permitted. Program enrollment and recruitment. Conduct program promotions. Adhere to strict procedures of confidentiality and HIPAA rules, release of information, and data security. Maintain accurate and complete participant records, tracking contact, counseling, testing, referrals, and outreach performance as defined by evidence-based models, local/state/federal entities and Inspiration and Change, Inc. Communicate effectively and professionally with partners and with clients for meetings, events, appointments, and follow-up on referrals. Maintain accurate and current process, procedural, and program, and inventory logs of specific outreach and testing activities, health materials, supply inventory, test kit temperature log (in a controlled storage area); and test results. Maintain and reconcile accurate forms and documentation. ADDITIONAL POSITION DUTIES: Conduct peer and educational outreach in person and via social media providing accurate and consistent health information and options. Communicate with leadership to improve the outreach strategy through weekly input and analysis of outreach activities. Provide culturally competent and culturally responsive services. Verify and document services received. Participates in other community events and activities that involve target populations to better deliver services. Conduct conversations about health and sexual health topics casually and comfortably. Participate in and help lead online events and social media live streams. Identify other online sites and brick and mortar venues where target populations can be effectively and efficiently reached. Provide timely follow-up with leads and individuals requesting information via our website, social media channels, or phone. Evaluate client needs to determine the best prevention or referral options. Assist clients in making appointments with healthcare providers. Document outreach activities and record relevant demographic information. Meet or exceed monthly goals and provide weekly reporting of program deliverables and goals. Maintain strict general confidentiality of clients. Ability to organize and manage a team of individuals from different fields, backgrounds, and diverse cultures. Technology-savvy, especially on mobile platforms. Experience with data collection and database systems. Strong written and oral communications skills in English and Spanish is helpful. Florida certified HIV tester (500/501). Training will be provided.
