Mission Statement

To enhance the health status of the citizen of North Central Wst Virginia.

Description

Support families in end-of-life care for their terminally ill loved ones in their homes through intermittent nursing visits. The patient has been certified by their physician to have a terminal diagnosis with a life expectancy of 6 months or less. An interdisciplinary team of nurses, social workers, home health aides, and volunteers work with the family and patient as a team to provide the care to the patient.
