The mission of the Green Tree Community Health Foundation is to improve the health status of community residents by initiating and supporting activities in response to identified needs in partnership with community resources.
Established in 2005, the Green Tree Community Health Foundation was created as a result of the sale of Chestnut Hill HealthCare to a joint venture between Community Health Systems, Inc. a for-profit health care system headquartered in Brentwood Tennessee and the University of Pennsylvania Health System.
Chestnut Hill HealthCare was an integrated health care system including the 100-year-old Chestnut Hill Hospital as well as Chestnut Hill Rehabilitation Hospital, and other related entities.
Green Tree Community Health Foundation is a not-for-profit public charity whose purpose is to improve the health and well being of communities of northwest Philadelphia and eastern Montgomery County. Through ongoing needs assessments, it identifies certain areas of vulnerability in these communities and provides funding to organizations whose work will address these needs. Additionally, the Foundation looks for opportunities to develop initiatives where it can fund and provide needed services directly.
The Green Tree Community Health Foundation was created to carry out the charitable outreach activities of the former not-for-profit Chestnut Hill Healthcare in its primary service area--an area extending from Nicetown to Norristown and from Manayunk to Ambler. The Foundation's focus is on these specific areas: The frail elderly; children and families; under and uninsured. Where appropriate, the foundation favors programs aimed at prevention.
