The Mission for the Homebound Elderly Program of Resurrection Home Health Services is to enable community residents to live independently, avoiding unnecessary/premature hospitalization or nursing home placement, by delivering comprehensive, high-quality home health products and services in a cost-effective manner.

Description

The driving force in the establishment of the Homebound Elderly Program was the desire to provide long-term in-home care to homebound frail ederly persons. This remains the heart of the agency today. Becasue the problems besetting the chronically ill elderely are often multiple and complex, the program offers a mix of medical, social and volunteer services. The agency's Homebound Elderly Program is one of the only prgrams in Chicago that provides medical care and social services on an ongoing basis, regardless of one's ability to pay.
