Description

Day Break III Respite and Caregiver Support Services, founded in November 7, 2001 offers a comprehensive array of supportive services to meet the needs of frail, homebound, and/or dependent seniors in Sunnyvale and other communities in North Santa Clara County.

These services include state-licensed adult day care, in-home respite care, escorted transportation, and high-quality informataion & referral services. Through these services, Day Break III aims to prevent premature or inapporopiate institutionalization of these elders, helping them maintain the highest possible level of independence so that they can remain in their family homes and in the community for as long as possible.
