LIFE at Home offers medical care and personal support to keep seniors active, healthy, and living at home. LIFE at Home is part of Pennsylvania's "Life" network of PACE facilities

Description

LIFE At Home is Pennsylvania's managed care program for frail elderly recipients who have been determined to need "nursing facility level of care" but wish to remain in their home and community as long as possible. LIFE provides a comprehensive all-inclusive package of services to meet their needs. The program is known nationally as the Program of All-inclusive Care for the Elderly (PACE)
