Mission Statement

In the tradition of our sponsor's commitment to honor human life and respect patient wishes, Improving Care through the End of Life will coordinate appropriate, supportive services for persons with life-threatening illness and a life expectancy of 12 months or less.

Description

We work with patients who are in the last year or so of their lives and have been identified by their doctors as people who should receive more attention than the staff at the clinics can provide.
