Description

Since 1896, VNA remains committed to its mission to improve the health status of the community, especially vulnerable populations: the very young, the very old, pregnant women, individuals with disabilities, the dying and the chronically or critically ill. The VNA is often the link between human service and health communities. We interpret how social conditions can impact the health of individuals, families or neighborhoods. We help the health and human service community understand how social issues impact vulnerable community members' ability to comply with the recommended plan of care. We help the individual and family access resources in both areas.
