State of the Heart Hospice serves our communities by providing individualized, exceptional care to patients and families experiencing a life-limiting illness, enhancing their quality of life.

Description

State of the Heart Hospice is a non-profit, patient-centered health care agency serving numerous counties in Western Ohio and Eastern Indiana. Our staff consists of Doctors, registered nurses, health aides, chaplains, volunteers, social workers, music therapists, and bereavement specialists. We provide expert pain control and symptom management in the patient's own home or any facility the patient considers home. Our social services team is there to support the patient and family during this time and after in grief support.
