Community Health Network of Connecticut Foundation, Inc. is a public, charitable organization. Formed in 2003 by Community Health Network of Connecticut, Inc., a not-for-profit managed care plan. The Foundation's mission is to advance, support, and promote programs and activities that fundamentally improve the health status of the people of Connecticut. This non-profit, charitable organization assists nonprofit activities of community health centers, as well as programs and activities that promote health care services to poor, distressed, and underprivileged individuals and families.
The Foundation is a volunteer-driven organization with an independent and non-funded Board of Directors who donate their time to the Foundation without compensation. The Foundation has been designated by the IRS as a 501(c) 3 entity, gifts to which are tax-exempt as charitable contributions.
These educational programs were designed to help community health centers develop new and improved models for quality health care delivery, disease management and disease prevention.
We look forward to partnering with institutions, health care facilities and community organizations that can help us develop programs that will produce solutions for Connecticut’s health problems. The Foundation envisions a state where all people have the ability to live healthy lives.
Our newest partnership is with Operation Frontline's Share Our Strength Program. CHNCTF is the first organization in Connecticut to adopt this program. Through the recruitment of volunteers, toegether we can help alleviate hunger, obesity and poverty.
