Description

AFPH is a 501(c)(3) non-profit organization that addresses chronic health disparities in underserved populations while enhancing the cultural competency of health professional students. It enables student volunteers to deliver culturally-competent cardiovascular preventive health services to the nation's poorest communities through an innovative model of screening; education; referral; and patient navigation. Since its beginnings in 2005, AFPH has trained hundreds of students and reached thousands of underserved patients.
