Portsmouth Community Health Center Inc. is a not for profit, community-based primary medical care organization dedicated to providing quality health care to medically underserved residents of Portsmouth, Norfolk, and the surrounding Hampton Roads communities without regard to their ability to pay.

Description

Portsmouth Community Health Center has two facilities: the original site located in Portsmouth, and the Park Place Medical Center in Norfolk. We are a JACHO accredited ambulatory care center, as well as offering the Health Smiles Dental Office, and Case Management Outreach services.
