St. Joseph's/Candler SOURCE (Service Options Using Resources in Community Environments) program seeks to positively impact the lives of older adults and younger people with disabilities through an innovative program of enhanced Case Management, support services and primary medical care. Through the coordination of these services, outlined by individualized care plans, SOURCE helps its members maintain good health and functional independence and avoid potential nursing home or other institutionalized care. We want people to live in their own homes as independent as possible for as long as possible.

Description

SOURCE is headquartered at the Georgia Infirmary which was created to give medical care for and to become one of the first teaching hospitals in the nation for African-Americans. Today our services and programs give much needed medical and supportive care, recreational and socialization activities to economically poor people beleaguered by unjust circumstances.

We commit our lives and resources to act in solidarity with the economically poor of the world seeking fullness of life and equality in society through one another as we embrace our multicultural and international reality. Our service and outreach efforts bring hope to those on the fringes of society. Traditionally, they are forgotten. Our mission is to make them the customer of choice.
