Mission Statement

Phases of Life, Inc. is designed to aid our community, by serving individuals experiencing mental impairment as part of the geriatric population, or those who require assistance with daily living. Those with diminished capacity due to health conditions or other impairments, are helped to maximize their potential to live and funstion in daily life. Bringing together program participants and their families in a unique setting through support with a full range of referral services, to help improve the quality of life. Our residential setting is appropiate for the needs of those who do not desire the "sterile" atmosphere that may exist with the traditional healthcare nursing facility.

Description

Our program give one to one support to participants, and provides the following:Close supervision 24 hours a dayAssistance with daily living need; eating drinkinh, hygeine and groomingWe provide job training and coaching for those individuals still of working age interested in entering the job market.Accompany residents to medical appointmentsProvide assistance with shopping,recreational and lesiure activitiesMedication managementPromoting positive images and an atmosphere of "can do" for those with Attention Deficit Disorder, Obsessive Compulsive Behavior, or Trauma to the brain.Respite ServicesIn- Home Care
