Mission Statement
PHSS supports individuals with complex medical, physical and/or developmental needs to live in their own homes and participate in community with family and friends. PHSS Support Services is a provincial, non-profit, community-based organization, whose mission is to support individuals with complex medical, physical and/or developmental needs, to live in their own homes and participate in community with family and friends. Its vision to be a leader in community inclusion through innovation and pushing boundaries, enables people to live in healthy caring communities, and take on the responsibility of contributing to the greater environment.
Description
PHSS provides support to more than 430 people at 80 locations in communities across Ontario, including London, St. Thomas, Stratford, Owen Sound, Ottawa, Nepean, Gloucester, Orleans and Lanark County. These support services include 24-hour residential support, day and overnight respite, specialized complex medical care, intensive behavioural supports, recreational programs, and assistance for those living at home with their families through the You At Home program. In addition to these services, people supported are encouraged to follow their passions, including through art, music, sport and other pursuits.
Our person-centered approach to care and support is the heart of everything we do. We understand the importance of placing the individual and their family at the forefront of their care journey and tailoring services to meet their unique needs and aspirations. Through this suite of individual services and supports, PHSS provides an alternative to institutional care. By offering home-based supports, we reduce the pressure on hospitals and institutions and in many cases, reduce the cost of care substantially. PHSS is committed to encouraging and creating an inclusive community where individuals with disabilities have access to all their community has to offer and that they are included in valued and meaningful ways.
