Community Care Hospice

Description

Community Care Hospice is primarily a home-based program, but services may also be provided in a nursing home or an assisted living facility. We provide and individualized program of physical, emotional, spiritual and practical care for people in the last stages of an incurable disease, with emphasis on control of pain and symptom management. The hospice volunteer is an integral part of the team.
Volunteers are carefully selected and are well-trained to support patients and families as they cope with terminal illness. As good listeners, nonjudgemental, adaptable, they have a strong desire to reach out with love and concern to others. Scheduling can be on a regular basis, or on call, depending on the needs and request of the patient and family.
