Empyrean Hospice

Description

Empyrean Hospice is a faith-driven organization that provides end-of-life comfort care in the home, for someone who has a life expectancy of six months or less. We are dedicated to caring for our patients in the same way we would care for our own loved ones, focusing on quality of life for whatever time remains. Our interdisciplinary care team of nurses, aides, social workers, chaplains and volunteers provides pain and symptom management, social and emotional support, and spiritual support for not only our patient, but also their family and caregivers.
