At our organization, we are dedicated to preventing and intervening in homelessness through an innovative approach. We developed a virtual Social Security Disability Case Management career training program that empowers individuals in their own communities by providing them with the education, skills, and professional experience needed to secure stable employment, ultimately contributing to their journey towards financial stability. Integrating proactive "boots on the ground" street outreach into the field training portion of our program allows us to reach those experiencing homelessness and mental health conditions directly within any city, addressing underlying barriers and providing a pathway to long-term housing solutions through access to health insurance, mental healthcare, and vital documents to support their Social Security Disability claims managed by our career training program participants.
