Mission Statement

The Transition Support Program exists to assist families that have successfully participated in/or completed the VGTH program to transition into the community with the intent of living self-sufficiently. This program supports the mission statementwhich says that VGTH will provide the broadest support possible to assist the VGTH families in reaching thier goals. The TSP will be financed independently of the VGTH program with the use of restricted money designated through reaching out.

Description

VGTH has set a goal to provide short term, supportive housing, which will allow the VGTH participant an oppurtunity to set goals and work towards an independent living situation. The VGTH staff will provide services and make the proper referrals with the community agencies. It is a two year transitional period for homeless or near homeless single parent families. The VGTH is an environment in which persons may establish themselves in a lifestyle of growing, self-worth, security and self-empowerment. Through linkage with the other agencies the VGTH would gain access to needed services, such as: job training, basic living skills, counseling, and other needed services. Working with the VGTH case manager, goals are set to establish the course of action the participant will choose for his/her life. We then do a three month follow up the participant after they leave the program.
