Working with diverse partners, CHN seeks to support the health and well-being of all. We carry out our mission as we: - Deliver community-based primary care, dental health, social services, and behavioral healthcare. Provide education and training to expand and diversify the healthcare workforce prepared to care for individuals, families, and populations in community-based settings. - Advocate for equitable healthcare access and treatment at the local, state, and national levels. Ensure patient access to health information to facilitate honest, transparent communication and seamless coordination of care. - Commit to anti-racism, equity, diversity, inclusion and belonging.
