CHN provides excellent, affordable, culturally responsive, inclusive, affirming, and equitable healthcare and social services that center individuals, families, and communities. Working with diverse partners, CHN seeks to support the health and well-being of all. We carry out our mission as we: - Deliver community-based primary care, dental health, social services, and behavioral healthcare. Provide education and training to expand and diversify the healthcare workforce prepared to care for individuals, families, and populations in community-based settings. - Advocate for equitable healthcare access and treatment at the local, state, and national levels. Ensure patient access to health information to facilitate honest, transparent communication and seamless coordination of care. - Commit to anti-racism, equity, diversity, inclusion and belonging.
Description
Community Healthcare Network (CHN) is a not-for-profit organization providing more than 65,000 New Yorkers with primary and behavioral healthcare, dental, nutrition, wellness, and needed support services.
Our network is made up of 14 federally qualified health centers throughout Brooklyn, the Bronx, Queens, and Manhattan, along with a fleet of mobile vans that bring health services to underserved people in need throughout New York City. We provide judgment-free, high-quality healthcare, without regard to race, religion, orientation, gender identity, immigration status or ability to pay. We turn no one away.
