Bristol Hospice, LLC, is graciously committed to our mission that all patients and families entrusted to our care will be treated with the highest level of compassion, respect and quality of care. The hospice model involves a team-based approach to expert medical care, pain management, and emotional and spiritual care expressly tailored to the patient's needs and wishes, while also supporting the patient's family and caregivers through the end-of-life process.

Description

Bristol Hospice programs provide hospice services to meet the physical, psychosocial, and spiritual needs of our patients and their families and caregivers. An interdisciplinary group of professionals and volunteers develops an individualized plan of care which includes, as appropriate, the following services:
> Pain and Symptom Management
> Counseling and Supportive Services
> Spiritual Counseling
> Therapy Services
> Personal Care/Home Health Aide Services
> Volunteer Services
> Supplies, Medication and Durable Medical
> Equipment Related to the Life-Limiting Illness
> Respite Care Services
